I acknowledge all our coaches; managers & board members have approved Risk Management; are in compliance with Michigan Sports Concussion Awareness Law; completed their CDC and completed the mandatory Safe Sports training and are familiar with current WMYSA rules and regulations.

We further understand that failure to comply with WMYSA Bylaws, Rules & Regulations may result in forfeiture of our club’s ability to participate in all state-run, sanctioned, and sponsored tournaments and programs.
