Affiliation Agreement

WMYSA Rules and Regulations state in an on-going fashion, member clubs shall update GotSoccer club accounts providing contact information of all club officers and indicating the registrar/community representative to the league. A club officer must submit the Affiliation Agreement in which the club agrees to abide by MSYSA and WMYSA Rules and Regulations as well as WMYSA Board policies, procedures and decisions. This will be done electronically each season and will be due at the same time as club commitments.

  4. President, Registrar, Treasurer and WMYSA representative names and email addresses
  5. Club billing address​​

  Affiliation statement indicating:

  1. Bylaws do not conflict with WMYSA bylaws
  2. You agree to adhere to WMYSA bylaws and rules and regulations,  policies and fee structure
  3. ,All coaches are familiar with rules and regulations and in compliance with state concussion law requirements.

Commitment information 

  1. Number of teams sponsored by WMYSA into MSPSP

The Club Affiliation Application is customized and sent to the President & Registrar. To receive membership approval and play in the fall season, the application and all supporting documents must be received no later than May 1st. To receive membership approval and in the spring season, the application and all supporting documents must be received no later than November 1st.
